PCHK Sponsorship Programme for Dental Care
Objectives:
- To provide financial assistance to those people who are staying in active employment for self-reliance to receive dental care and treatment
- To allow the people in need to receive dental treatment in time so that they would not put off treatment for their dental problems
- In the long term to increase the awareness of oral health in the community
Target beneficiary:
- Non-CSSA recipient and working low income people
- Retired person
Eligibility Criteria:
- Meet the income and asset limits (property occupied as dwelling is not included)
- If eligible person is aged 65 or above, his/her healthcare voucher amount balance must be $0.
Content:
- To be referred by dentists of our clinics upon oral consultation
- Sponsored items included minor oral surgery, root canal treatment, denture, crown and bridge
- A few quotas will be allocated each year to subsidize the eligible persons for a 50% sponsorship of the treatment cost with a ceiling amount.
Application Procedures:
- Visit our dental clinics for oral consultation and referred by dentists for this sponsorship programme
- Fill in application form and submit together with the copies (the latest 3 months) of income proof (if any), savings account passbook and bank statement
- Sign a declaration statement to self-declare the eligibility
- Submit the application form and all document proof to Shop 116, G/F, Lei Cheng Uk Arcade, Shamshuipo, Kowloon OR any dental clinics of Project Concern Hong Kong
